Berks food safety inspections

Berks County is divided into three jurisdictions: Reading, Muhlenberg Township and the rest of the county. Reading and Muhlenberg Township have their own food safety inspectors and the rest of the county is inspected by Pennsylvania Department of Agriculture inspectors.

The results will be posted every other week. All food vendors are inspected at least once a year. Other inspections can be triggered by a change of owner, a follow up if a facility had been out of compliance or by a complaint filed with the PDA.

If a food provider is labeled out of compliance, that means it has one or more violations that require a return visit by an inspector. The establishment is given time to fix the problem(s) and remains open for business.

Below are the results of inspections conducted between Nov. 9 and Nov. 22, 2022, that were filed in the state database as of Nov. 23 at 3:30 p.m.

Cold Stone Creamery #1288, 2747 Paper Mill Road Suite Z1, Wyomissing, Nov. 21: two violations. Two spatulas located in the back area are not in good repair or condition. One of the sliding doors on the ice cream display case is cracked/damaged.

Dollar General #15647, 15 N. Centre Ave., Leesport, Nov. 21: no violations.

Hollywood Casino Morgantown, 6021 Morgantown Road, Morgantown, Nov. 21: five violations. Observed organic debris on the exterior surface of the ice machine in the Tony Luke kitchen. Cheese sauce, a commercially processed food was only heated to 115°F before being placed in the steam table and not to 135°F for hot holding as required. Corrected. Roasted chicken wings were held at 48°F, in the cook line area of Barstool Sports, rather than 41°F or below as required. Corrected. Observed numerous storage pans with old label residue in the clean dish storage area throughout the facility. Observed single service containers in the Red Lotus front counter area not covered or inverted.

Van's Cafe, 506 Penn Ave., West Reading, Nov. 21: three violations. An insect control device (fly strip) is located in the front area with potential to contaminate food, equipment, and / or utensils. Hood surfaces have an accumulation of grease. One set of lights in the food preparation area is not shielded.

Barto Shell, 1325 Route 100 Barto, Pa 19504, Nov. 18: no violations.

Friendship Hook & Ladder Fire Co #1, 10 Warwick St., Boyertown, Nov. 18: no violations.

Giovanni's Pizzeria, 1306 Reading Ave., Boyertown, Nov. 18: five violations. Observed cooked, ready-to-eat bacon, stored on top of a case of raw chicken breast tenders in the walk-in cooler. Corrected. Observed raw hamburger patties stored above ready to eat foods in a reach in cooler in the food prep area. Corrected. Food dispensing utensil in bulk spices observed stored in the food and not with handle above the top of the food and the container. Corrected. Scoop being stored in the ice machine with handle touching and/or buried beneath the ice. Corrected. Observed excessive food and grease debris on the exhaust fan and surrounding wall and floor in the food prep area. Floor in the food prep area is made of is cracked/roughened/and is not a smooth, easily cleanable surface. Observed wet wiping cloths in the food prep area, not being stored in sanitizer solution. Observed wet wiping cloths used to secure a cutting board on a prep table.

Great Wall Chinese Restaurant, 532 Penn Ave., West Reading, Nov. 18: one violation. An open employee's beverage container was observed in a food preparation area. Corrected. Food employee eating in kitchen area as evidenced by observed partially consumed food on a food preparation table. Corrected.

Greenhouse Cafe, 18 State Hill Road, Wyomissing, Nov. 18: two violations. Food employee observed in kitchen area, not wearing proper hair restraint, such as nets or hats. Ingredient list is not available for the vegan baked goods offered for sale at the front counter.

Redner's Warehouse Market #87, 1179 Ben Franklin Hwy Douglassville, Pa 19518, Nov. 18: four violations. The heat pads on the plastic wrap machines in the deli and meat areas were observed to be in poor condition and no longer a smooth easily cleanable surface. Observed old food debris in the grates and shelving at the leafy greens area of the produce display case. Observed heavy dried milk residue on the milk shelving in the sales floor cooler. Floor in the deli walk-in cooler is damaged/cracked/roughened and is no longer a smooth, easily cleanable surface. The wall paneling in the walk in cooler prep room is damaged and not an easily cleanable surface.

River Rock Academy @ Amity Primary Center, 2144 Weavertown Road, Douglassville, Nov. 18: one violation. Soap was not available at the handwash sink in the food prep area. Corrected.

Third And Spruce Drafthaus, 238 S 3rd Ave., West Reading, Nov. 18: three violations. Observed an accumulation of old food debris on the floor under some equipment and a set of shelves in the cooking area. Working containers (spray bottles) in bar and kitchen areas, used for storing chemicals, cleaners taken from bulk supplies, were not marked with the common name of the chemical. Corrected. Food facility does not have available sanitizer test strips to determine appropriate sanitizer concentration (for the glass washer, dishwasher, and 3-compartment sink).

Weidner's Genuine Jerky, 12 S. Reading Ave., Boyertown, Nov. 18: one violation. Containers of various chemicals were stored above or on the same shelf with food, equipment, and/or single service articles in the back prep area. Corrected.

West Reading Tavern, 606 Penn Ave., West Reading, Nov. 18: no violations.

Wyomissing Junior/Senior High School, 630 Evans Ave., Wyomissing, Nov. 18: no violations.

Arbys #7714, 809 S 4th St., Hamburg, Nov. 17: three violations. Soda dispenser nozzles, a food contact surface, were observed to have mold like residue and was not clean to sight and touch. Corrected. The handwash sink located in the drive thru area does not have water at a temperature of at least 100°F. Corrected. Observed food debris build up on shelves and equipment throughout the food prep area of the facility.

Berks Career & Technology Center-West Campus, 1057 County Welfare Road, Leesport, Nov. 17: no violations.

Candy's Homemade Ice Cream, 1085 Pottsville Pike, Shoemakersville, Nov. 17: two violations. Food dispensing utensil in ice cream toppings, observed stored in the food and not with handle above the top of the food and the container. Floors in the prep area is cracked, missing, and is not a smooth, easily cleanable surface.

Chef David's Food & Friends, 105 Evans Ave., Wyomissing, Nov. 17: two violations. Bare wood studs (2×4's) used for the divider wall are not smooth, non-absorbent. The food facility does not have the original certificate for the certified food employee posted in public view.

Cyber Cafe @ Psu Berks (Gaige Bldg), 2080 Tulpehocken Road, Wyomissing, Nov. 17: no violations.

Dawn's Deli The Next Generation, 71 Penn St., Lenhartsville, Nov. 17: one violation. Raw shell eggs stored above and on the same shelf as ready to eat foods. Corrected.

Domino's Pizza, 1350 Pottsville Pike, Shoemakersville, Nov. 17: no violations.

Nirvana Indian Bistro, 1137 Berkshire Blvd., Wyomissing, Nov. 17: four violations. Food facility has an employee who held a Certified Food Manager certificate; however, the certificate has expired and is no longer valid. Bulk food ingredient storage containers in the kitchen area are not labeled with the common name of the food. Food dispensing utensils in bulk food bins observed stored in the food and not with handle above the top of the food and the container. Corrected. Observed some containers of food being cooled directly on the floor in kitchen area, rather than 6 inches off of the floor as required. Corrected.

Sushi With Gusto At Redner's #77, 1149 Berkshire Blvd., Wyomissing, Nov. 17: no violations.

Thaiwat Restaurant, 1145 Berkshire Blvd., Wyomissing, Nov. 17: two violations. Observed in-use knife stored between table edges, an area not easily cleaned & sanitized. Corrected. Observed some food items stored directly on the floor in the walk-in cooler and walk-in freezer, rather than 6 inches off of the floor as required.

Tully's @ Psu Berks (Perkins Bldg), 2080 Tulpehocken Road, Wyomissing, Nov. 17: one violation. One of the light fixtures is not shielded or shatter proof in the front food preparation area. Light guard has been ordered.

Bake Shop At Skyline Drive Orchard, 118 Skyline Drive, Reading, Nov. 16: one violation. Food facility is not using an approved detergent-sanitizer in both compartments of the two compartment manual warewashing sink.

Carlo's Italian Restaurant, 1147 Penn Ave., Wyomissing, Nov. 16: two violations. A food employee was observed touching a sandwich roll — a ready-to-eat food — with bare hands. Corrected. The following areas have an accumulation of dirt and/or old food debris: the floor under the fryers, the floor under the shelving in the walk-in cooler, and the shelving in the walk-in cooler.

Daniel Boone School District Primary Center, 576 Monocacy Creek Road, Birdsboro, Nov. 16: no violations.

Little Nicky's Pizza, 31 Douglassville Shopping Center, Douglassville, Nov. 16: five violations. Meatballs in sauce, which was cooled, was only reheated to 110°F for hot holding and not 165°F for 15 seconds as required. Corrected. Tape and cardboard are being used to repair and insulate the handles of the pizza ovens. These materials are unapproved for food equipment. Manual warewashing equipment observed with build up of filth and food residue and not cleaned before use, and frequently throughout the day. Food dispensing utensil in tomato sauce observed stored in the food and not with handle above the top of the food and the container. Corrected. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration.

Michelangelo's, 500 Penn Ave., West Reading, Nov. 16: no violations.

Starbucks Coffee #62517, 911 Heritage Drive, Elverson, Nov. 16: three violations. Observed excessive food debris build up on shelving and floors at the front counter area. A working container of cleaner sanitizers were stored above or on the same shelf with food, equipment, and/or single service articles in the back food prep area. Corrected. Paper towel dispenser empty at the handwash sink in the back food prep area. Corrected.

Taco Bell #36257, 1136 Ben Franklin Highway, Douglassville, Nov. 16: no violations.

Fleetwood Area School District Willow Creek Elementary, 605 Crisscross Road, Fleetwood, Nov. 15: no violations.

Pour House American Grille, 3341 Pricetown Road, Fleetwood, Nov. 15: 16 violations. Food employees observed eating or tasting food in the kitchen area. Open employee beverage containers were observed in the kitchen food preparation areas. Digital thermometer for the Hoshizaki 2-door refrigerator is not readable and there is no other thermometer available in this unit. Thermometer for ensuring proper temperature of equipment is not available or readily accessible in the walk-in cooler in the basement. The slicer and potato slicer, food contact surfaces, were observed to have an accumulation of dried food residue and were not clean to sight and touch. One of the soda guns at the bar, a food contact surface, was observed to have dark residue and was not clean to sight and touch. The interior of the ice machine, a food contact surface, was observed to have dark residue and was not clean to sight and touch. The food facility does not have the original certificate for the certified food employee posted in public view. Certificate is in the kitchen, and will expire 11/30/2022. Working container (spray bottle of blue liquid) in serving area, used for storing chemicals, cleaners taken from bulk supplies, was not marked with the common name of the chemical. The Person in Charge does not have adequate control of food safety in this food facility as evidenced by this non-compliant inspection. Observed an accumulation of dirt/old food debris on the floor near the slicer in the basement. Wall at the food preparation table in the side room is not clean to sight and touch. A food employee was observed preparing vegetables for salads — ready-to-eat foods — with bare hands. Corrected. Cleaned and sanitized utensils located in kitchen area are stored with food contact surfaces/food or lip area exposed to hand contact when retrieved by food employees. Corrected. The handwash sink in the bar area was blocked by a container and was not accessible at all times for employee use. Corrected. The handwash sink at the cooking line is blocked by a shelving unit and is not accessible at all times for employee use. The bulk food ingredient storage containers in the basement area are not labeled with the common name of the food. Scoop being stored directly on top of the ice machine in the basement. Observed in-use knives and /or cleavers stored between table edges, an area not easily cleaned & sanitized. Corrected. Observed a bag of carrots and a bucket of onions stored directly on the floor in the walk-in cooler, rather than 6 inches off of the floor as required. Also observed several cases of food stored directly on the floor in the walk-in freezer. Observed a bucket of sliced potatoes and a bucket of shrimp in the mop sink in the kitchen. Corrected. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. A call was placed for service for the dishwasher and this technician corrected this issue prior to the end of this inspection. The floor / wall juncture in the dishwasher area is not coved and closed to 1/32 inch (coving is missing). The light in the basement walk-in cooler and one of the bulbs in the hood are not shielded or shatterproof. Out of compliance.

Antietam Mt. Penn Elementary Center, 2310 Cumberland Ave., Reading, Nov. 14: one violation. Box fan at the right-hand serving line has an accumulation of dirt/dust (fan plugged in and directed at serving line, but was not running during this inspection).

Antietam School District Mt Penn Primary Center, 201 N. 25th St., Reading, Nov. 14: no violations.

Antietam Senior High School, 100 Antietam Road, Reading, Nov. 14: one violation. Outer door located near the walk-ins in the kitchen has a gap at the bottom and does not protect against the entry of insects, rodents, and other pests.

Arbys #1878, 4901 Perkiomen Ave., Reading, Nov. 14: two violations. There are several missing or damaged coving tiles throughout the kitchen and storage areas. Observed an accumulation of dirt and old food debris on the floor in the following areas: under equipment at the drive-thru area, near the ice machine, near the mop sink, and at the dishwasher/3-compartment sink.

Mamas Famous Pizza Exeter, 35 W. 47th St., Reading, Nov. 14: two violations. The handwash sink at the front counter area was blocked by a sanitizer bucket and was not accessible at all times for employee use. Corrected. Open employee beverage containers were observed in the walk-in cooler and at the pizza prep station. Corrected. Food employees eating in kitchen area as evidenced by observed partially consumed cookies in the kitchen area. Corrected.

Wendy's # 19312, 5411 Perkiomen Ave., Reading, Nov. 14: two violations. A partial flat of raw eggs was stored above ready to eat foods in the walk-in cooler. Corrected. Thermometer for ensuring proper temperature of equipment is not available or readily accessible in the small refrigerator at the drive-thru window. Corrected.

Countryside Eatery, 15382 Kutztown Road, Kutztown, Nov. 10: no violations.

Joe's Kwik Marts (#1045) Exxon, 2855 Perkiomen Ave., Reading, Nov. 10: two violations. Food facility has lost its certified food employee over 3 months ago and has not replaced the certified food employee as required. Roof in the storage area behind the front counter is leaking water and is in need of repair.

Kutztown School District Elementary School, 40 Normal Ave., Kutztown, Nov. 10: one violation. The pedestal fan being used in the dishwasher area has an accumulation of dust/dirt.

Kutztown School District Middle School, 10 Deisher Lane, Kutztown, Nov. 10: no violations.

Kutztown Area School District High School, 50 Trexler Ave., Kutztown, Nov. 10: no violations.

Liberty At Dryville, 110 Lyons Road, Mertztown, Nov. 10: eight violations. Observed ice used as a coolant in the waitress station, being used as food. Pan of lemons stored in drink ice. Corrected. Preset tableware is not properly wrapped to cover eating area of clean and sanitized utensils. Observed vacuum-packaged salmon being thawed in packaging which is a prohibited thawing method. Corrected. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in reach in cooler under bar counter equipment. Toilet rooms do not have a self-closing door. Womens toilet room closer broken and must be repaired. Dry breading mixtures used for dipping of raw animal foods is not sifted every 4 hours to remove excess moisture and doughballs which may render the dry mixture a TCS food per FDA guidance document issued 6/28/04. Corrected. Observed clean bowls in dish rack area, stored uncovered or not inverted that have old water in them. (wet nested) Old labels identifying a different product on some current food items in reach-in refrigerators.

Olivia Produce, 10 S. Summit Ave., Reading, Nov. 10: four violations. Observed very deeply scored cutting boards not resurfaced or discarded as required. Cut produce food, in the display area, is a packaged food possibly subject to water entry and is stored or displayed in direct contact with ice. Corrected. Walk-in cooler fan box/ceiling area of the food facility is extremely dirty, dusty, and in need of cleaning. Also some shelving units have dust/dirt build up. A working container of cleaner/Draino and sanitizer was stored above or on the same shelf with food, equipment, and/or single service articles in the kitchen area. Corrected.

Saint Ignatius Loyola School, 2700 St. Albans Drive, West Lawn, Nov. 10: two violations. Observed milk cartons stored directly on the floor in walk in cooler area, rather than 6 inches off of the floor as required. Prerinse hose spray nozzle for the 2 compartment warewashing sink is chipped/peeled and no longer a smooth easy to clean surface.

Salino's Stand, 10 S. Summit Ave., Shillington, Nov. 10: two violations. Observed clean bowls in clean dish area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). Observed clean food equipment and/or utensils in kitchen area, stored uncovered or not inverted. Commercially processed refrigerated, ready to eat, time/temperature control for safety food, located in the customer reach in refrigerator, is not being marked with a use by/discard date.

Anthony's Coal Fired Pizza, 2733 Papermill Road, Wyomissing, Nov. 09: five violations. Open employee beverage containers were observed near the pizza area and at the bar. Corrected. Scoop at the bar being stored in the drink ice with handle touching the ice. Corrected. Door located in the back area of the food facility has a gap along the side and does not protect against the entry of pests. Side seal is deteriorated. Food facility hot water heater is not producing enough hot water to supply some of the sinks at the time of this inspection. Facility has already initiated a service call for this issue. Observed a buildup of dirt and old food debris on the floor under equipment in the front and back areas of the facility.

BBQ & Eggacy LLC At Leesport, Leesport Farmers Market, Leesport, Nov. 09: four violations. Take-out containers in the Eggacy area not stored inverted to protect the food contact surface. Corrected. The food facility does not have the original certificate for the certified food employee posted in public view. Certificate is available but not posted. The handwash sink was blocked by a sanitizer bucket and was not accessible at all times for employee use. Corrected. Observed water leaking at the 2-compartment sink faucet.

Bixler's Country Meats, Leesport Farm Market PO Box 747 Bldg 1, Leesport, Nov. 09: no violations.

Breakfast Hut, 4170 Perkiomen Ave., Reading, Nov. 09: two violations. The handwash sink in the cooking area does not have single use towels. Corrected. Lights are not shielded or shatter proof in the dry storage area.

Governor Mifflin School District Cumru Elementary, Philadelphia Ave., Shillington, Nov. 09: one violation. Observed raw shell eggs stored above ready to eat foods in the walk-in cooler. Corrected.

Red Plate Diner, 440 E. Penn Ave., Wernersville, Nov. 09: three violations. Assorted desserts/milk products were held at 50°F, in the front counter area, rather than 41°F or below as required. Items discarded as necessary and some items put on ice. Observed cardboard being utilized as a floor covering in walk in cooler. Corrected. The food facility does not have the original certificate for the certified food employee posted in public view.

Speedway #06771, 328 E. Penn Ave., Wernersville, Nov. 09: four violations. Observed soda fountain nozzle and coffee creamer dispenser equipment, in store area, with an accumulation of dust, old splash residue around nozzle and splash guard surfaces. Drain lines in the drip tray of the soda fountain are clogged and stagnant/slimy water is built up in tray. Lights are not shielded or shatter proof over the hot pizza holding case. Ceiling area in entrance of the walk in cooler of the food facility is extremely dirty, dusty, and in need of cleaning.

The following reports for the period of Oct. 26 to Nov. 8, 2022, were added to the state's database after Nov. 11 and did not appear in the previous Berks food safety report.

Chuck E Cheese #539, 2721 Meridian Blvd., Wyomissing, Nov. 8: one violation. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was <10 ppm, and not 50-100 ppm as required. Corrected immediately by priming the sanitizer pump.

Fresh Breakfast/Lunch, 614 Penn Ave., West Reading, Nov. 8: one violation. Sanitizer test strips for the 3-compartment sink are deteriorated and need to be replaced. Facility also needs a method to verify the temperature at the hot water sanitizing dishwasher.

Gino's Cafe, 400 W. Broad St. Unit D, Shillington, Nov. 8: five violations. Glass containers of sauce offered for sale at counter do not have a standard of identity or facility info on jar. Food facility is using Chlorine Sanitizer at an extremely high concentration of 200ppm, instead of 50-100 ppm. as approved in the Code of Federal Regulations for food contact sanitizing. Plastic soda cups in dining area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). Observed in-use knives and /or cleavers stored between table edges or between tables, an area not easily cleanable & sanitized. Corrected. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Walk-in cooler floor has dirt buildup around corners of walls.

Gouglersville Fire Company #1, 475 Mohns Hill Road, Sinking Spring, Nov. 8: six violations. Ceiling tiles missing in the kitchen area, and need to be replaced. Walk-in cooler has food debris/spillage under shelves and must be cleaned more frequently. A working container of grill cleaner was stored above or on the same shelf with food, equipment, and/or single service articles in the rear kitchen area. Corrected. Observed in-use knives and /or cleavers stored between table edges or between tables, an area not easily cleanable & sanitized. Observed hood vent equipment area, above dishwasher with an accumulation of dust, dirt, debris on non-food contact surfaces. Food facility has an employee that has taken food safety training program; however, the food safety program was not an ANSI/CFP accredited Certified Food Manager program.

Target #1222, 2769 Papermill Road, Wyomissing, Nov. 8: three violations. The food facility does not have the original certificate for the certified food employee posted in public view. A working container of Febreeze was stored above and on the same shelf with food, equipment, and/or single service articles in the back kitchen area of the snack bar. Corrected. Ice bucket in the back kitchen area of the snack bar is cracked.
